• Explain what work skills might be impacted by your disability.
• Tell us what supports you might need.
• Must be signed by a physician, either on their letterhead or with their contact information.

Certain jobs in our Business Solutions department require employees with disabilities to provide these forms. If you apply for one of these jobs, our case workers will help you get the paperwork you need.

Many of our Business Solutions employees work on contracts through SourceAmerica and the AbilityOne program. These jobs are reserved for people with a severe disability. The disability documentation helps us to see if you qualify. It also helps us learn how we can better support you, so you can succeed in your new job!
